[Audio] This memo is to inform you that the " Staff Incident Log" form will be completed by all Residential Home Supervisors and the Residential Generalist for incidents concerning residential and Delta-T staff. The "Staff Incident Log" form will be turned in on Thursdays for the night supervisor, and on Fridays for the other non-night supervisors. The staff incident logs need to be submitted before two PM Thurs or Fri by in-person electronic typed submission or email. Steps: One – If there is an issue to report. Electronically type the incident on the log form. Two – Email document(s) to the Program Manager. If the incident occurs after 3 pm. Write the situation on the incident log and submit the document once, it's written. Three –The Residential Program Manager will provide confirmation the document was received and provide feedback if needed. Failure to report staff incidents will result in a write-up and retraining of the purpose and use of the Staff Incident Log (no exceptions). Leaders, it is your responsibility to ensure all staff incidents are recorded, and the staff that's not performing to standard or below standard will be notified of their need approvements when the infraction is noticed. Failure to follow the above directives concerning checking daily/weekly electronic/physical documentation will result in disciplinary action up to separation from your employment with Denton County MHMR if the issue continues to occur after corrective actions have been discussed and implemented.
