Patient Background. 85 y/o M, living with adult son Wife is deceased Medical Hx: prostate cancer 20 yrs ago, 15-year hx of HTN & hypercholesterolemia Surgical Hx: Prostatectomy Social: 55 yr hx of smoking & alcohol, now sober with minimal smoking High level of independence – requires assistance for transportation Medications: Hydrochlorothiazide, atorvastatin.
Community support & Activities. Family assistance with: Meals Errands Shopping Transportation Family members involved: Sons (x3) Daughter Grandchildren.
Fluid volume deficit r/t inadequate fluid intake.
Risk for falls r/t medications. Side effects that contribute to risk: Hypotension Dizziness – reported by pt Weakness – reported by pt Drowsiness Risk for eye problems w/ hydrochlorothiazide.
Risk for decreased cardiac output r/t htn. S&S to assess for: Tachycardia Changes in BP Diminished peripheral pulses Diaphoresis Cool skin JVD Edema Chest pain Changes in urinary output.
