Location – Community project held (San jose n.d ) Population – Density, Age, Ethnicity , Language ( united states Census, n.d ) Work and entertainment (san jose , n.d ) Jobs opportunity, Recreational parks, bars and Restaurants Healthcare opportunities- Hospitals, medical clinics, dental clinics. Homes and Schools – Single Family house, School activities and sports.

COMMUNITY DATA ASSESMENT. What problems we faced in our community. Lack of knowledge Language barrier.

COMMUNITY HEALTH DIAGNOSIS. The nursing diagnose is Deficient knowledge regarding hypertension and diabetes related to lack of information as evidence by unhealthy lifestyle.

DEVISED COMMUNITY HEALTH PLAN. Partner and resources were used to achieve goals. Educate them about disease from videos and teach them what precaution can they take Provide pamphlets where they can learn about low sodium diet and diabetes diet. EVALUATION. Goal was to report change in their Blood pressure or BS level. Questionnaire were filled to check their progress Daily activity report were submitted..
