[Audio] Sexual Harassment Policy Statement Grae-Con Construction, Inc., Special Installations, Grae-Con Process Piping, LLC, and Grae-Con Electric, Inc. are committed to provide a work environment in which all individuals are treated with respect and dignity. Further it shall be understood by all employees that sexual harassment in the workplace is in violation of our company policies. Sexual harassment constitutes discrimination and is illegal under federal, state and local laws. For the purposes of this policy, "sexual harassment" is defined, as in the Equal Employment Opportunity Commission Guidelines, as unwelcomed sexual advances, requests for sexual favors, and verbal or physical conduct of a sexual nature will be constituting harassment when submission to such conduct is made a term or a condition of an individual's employment. • When submission or rejection of such conduct is used in employment decisions affecting the individual involved, or; • When such behaviors have the effect of interfering with an individual's work or of creating an offensive work environment. It is our policy to take corrective actions should acts of sexual harassment in the workplace occur. Believing that prevention is the most appropriate activity in seeking the elimination of sexual harassment, it is our policy to express our rejection of such behavior on an annual basis. All employees shall know of their right to raise the issue of harassment with either, the General Superintendent, Project Superintendent, or Corporate Officer. For employees at project sites, grievances need to be directed to the highest rank management available, who has the responsibility for carrying the issue to the President for resolution. [Audio] Grae-Con General Safety Policy All employees including management of Grae-Con are expected to follow the Safety and Health policies and programs. All supervisors will strictly enforce the Safety and Health policies and programs. Training will be provided to ensure that all policies and procedures are followed. PPE Grae-Con minimum requirements Hard Hat Safety Glasses Safety Toe Boots Other PPE will be required for specific tasks and will be covered at the STA meeting Submitting to Drug and Alcohol testing (including random) is a condition of employment.

[Audio] Anyone involved in Horse play is subject to disciplinary action. If you don't know how to do something or need help, ASK. Report All accidents no matter how minor. Report All unsafe conditions regardless how it was created. The workplace will be inspected daily for hazards prior to commencing work. All relevant OSHA and other federal regulations will be adhered to Toilets/ facilities, first aid, fire extinguisher locations will be addressed to all employees Emergency and Evacuation Plan (know where the muster areas are) Good Housekeeping maintained throughout the duration of the job Sexual harassment, Worksite Violence, drugs, alcohol, fire arms/ weapons are prohibited on All Grae-Con sites. Cell phone use in the field is limited to supervisor, safety and management.

[Audio] Understanding Haz-Com for safety of all workers. All workers will attend and sign Weekly Safety meeting sheet. Employees will be trained or assigned to tasks they are already trained for. Safety Data Sheets and the Safety Program manual will be on site. There shall be a trained First Aid responder and First Aid Kit on site. Seatbelts are to be worn in all vehicles and equipment while in operation Inspected and fully charged fire extinguishers will be on site. Only approved flammable containers will be used on site. All rigging will be inspected before use and any defective items will be removed from service immediately All power hand tools will be inspected before use and removed if defective.

[Audio] All temporary electrical services shall have a GFCI No electrical tools or equipment will be used in hazardous or wet conditions unless it is designed for that use All ladders will be of proper length and inspected for defects All floor and wall openings shall be protected by guard rail, mid rail and toe board or covered and marked with material to handle the load. All stairs shall have standard hand rail/ guard rail. All equipment to be worked on shall be LOTO or put in 0 mechanical state Confined Spaces ( tanks, ducts, etc.) shall be air monitored and the workers will be trained. The confined space program shall be followed and permitted confined space work areas are to be equipped with a rescue team/ plan onsite..

[Audio] Grae-Con Disciplinary Policy All personnel on the site are co-workers. In order for us to maintain the best working conditions, all must work together on safety and health issues. The suppliers bringing materials to the site will also abide by our policy to maintain a safe workplace. Always watch out for yourself and your co-worker's safety. When Safety procedures/ precautions are violated, responsibilities are not met, accidents, injuries, property damage, drugs and alcohol violations or any other act that destabilizes the integrity of safety on our sites or has adverse ramifications to Grae-Con, the Disciplinary Policy will be enforced. The Disciplinary Policy is as follows. 1st Offense verbal warning, 2nd Offense written reprimand, 3rd Offense is termination. The Superintendent, Upper management, Safety are responsible for enforcing the safety policies and may alter the disciplinary policy at their discretion with just cause. (i.e. frequency, severity of offense, IDLH violations, etc.). Supervisors/ Upper Management/ Safety will fill out the appropriate form. (i.e. Reprimand, Termination Form).

[Audio] Worker Safety Roles & Responsibilities Report to work Be fit for duty Be ready to work and with a good safety attitude. Become familiar with safety programs and abide by policy. Perform ALL duties with Safety in mind Use all your PPE that is necessary and or required Report all unsafe conditions and/ or suggest a safer approach to job Report All accidents no matter how small. Be alert to the safety of your surroundings and coworkers Attend All weekly safety meetings and specific task training. Inspect all tools and equipment you are to use. If you are unclear of your Task, ASK. (It will be safer for everyone) Follow the Site Safety Task Procedure (aka Safe Job Procedure in site plan).

[Audio] Fall Protection Grae-Con General Policy is you must use Fall Protection when working 6 feet above the lower level. ( Site Specific may be more stringent) Fall Protection systems may include but are not limited to: Top rail/ guard rail, mid rail and toe boards Barricades Personal Fall Arrest Systems Anchorage point ( 5000 lb.) Connector ( lanyard, retractable, etc.) Harness Warning Line Systems Controlled Access Zones Safety Monitoring system Hole Covers Workers will be trained in the Fall Protection Plan.

Shirts must be at a length to cover Chest & Torso Shirt Sleeves must also have a minimum length of 4inches Gloves (The appropriate glove for the assigned task) Hearing Protection (As needed) Face Shield (when grinding, using tools that may cause flying debris, etc.) FR Clothing (when welding, Client required, performing hot work, etc.) Respiratory Protection (When hazards are above the PEL) Cover All, Tyveks, etc. (When protection is needed above standard clothing) Other PPE may be required (When assigned to use for a specific task or site specific).

[Audio] Complacency Steps to avoid complacency: Take a minute to think about the task Plan the job Is there a better and safer way to do this task Do you have everything you need Redefine your approach to the task if at all possible Take a break and look around at your coworkers (are they being safe) Try to avoid or reduce repetition as much as possible Interact with co workers to help them and ask for help Get plenty of rest before coming to work Remember any close calls if you performed this work before Don't let your guard down because things are going well Walk with your head up and looking around Do not allow yourself to daydream when performing a task Don't dwell on the negative.

[Audio] Stop Work Authority Stop Work Authority should be initiated for conditions or behaviors that threaten danger or imminent danger to people, equipment or the environment. Anyone who recognizes the hazards below will have Stop Work Authority. Situations that warrant a SWA may include, but are not limited to the following: Alarms Change in conditions Changes to scope of work or work plan Emergency situation Equipment/ Tools used improperly Lack of knowledge, understanding or information Near-miss incident Unsafe conditions.

Immediately determine if there is an injury or illness Secure the area, hold equipment/ property for inspection & take photos .Fill out the Quick Incident Report Superintendents will immediately send Quick Incident Report to the Corporate Office by phone at ( 740) 282- 6830 ( Dan, Randy and/ or Rob). Begin the investigation process by having employee fill out their section and the superintendent fill out theirs of the Incident Investigation Report The Incident Investigation Report is in the Grae-Con Safety Manual. Call the emergency squad or arrange for medical transportation and evaluation if needed. If in Doubt call the emergency squad. ( Drug & Alcohol test to be administered at medical facility).

[Audio] Information needed and available at time of call to office: (Use Incident Quick Check Sheet from Grae-Con Safety Manual) How many are injured Name of injured person(s) and the company they are working for What part of the body was injured Severity of injury Medical attention required Where they are sent to What, When and Where it happened When it was reported and to Whom it was reported Names of Witnesses Investigate the incident. Obtain names of all involved parties and witnesses. Record the names of other companies/ their personnel involved (if any). Do not speculate or discuss accident with persons not involved. Complete an accident investigation form and submit it to the main office. Immediately implement corrective action to assure that there is no reoccurrence Safety Audit is to follow within 24 hours. The Regional VP, General Superintendent, Project Superintendent, Regional Safety and Safety Director are mandatory in the investigation.

[Audio] Scaffolding Erected or directly supervised by " Competent Person" Scaffold User training will be done. First thing a user should check to see what color the scaffold tag is. If it is signed and dated. Self-supporting scaffolds ( Tube, Frame, Mobile, etc.) The footing or anchorage for scaffolds shall be sound, rigid, and capable of carrying the maximum intended load without settling or displacement. Unstable objects such as barrels, boxes, loose brick, or concrete blocks shall not be used to support scaffolds or planks. Guardrails and toe boards shall be installed on all open sides and ends of platforms more than 10 feet above the ground or floor, except needle beam scaffolds and floats. Scaffolds 4 feet to 10 feet in height having a minimum horizontal dimension in either direction of less than 45 inches shall have standard guardrails installed on all open sides and ends of the platform. Guardrails must be 2 X 4 inches, or the equivalent, not less than 36 inches or more than approximately 42 inches high, with a mid rail, when required, of 1 X 4 inch lumber, or the equivalent. Supports must be at intervals not to exceed 8 feet. Toe board and the guardrail shall extend along the entire opening..

Any scaffold, including accessories such as braces, brackets, trusses, screw legs, ladders, couplers, etc., damaged or weakened from any cause must be repaired or replaced immediately, and shall not be used until repairs have been completed. All planking or platforms must be overlapped (minimum 12 inches) or secured from movement. An access ladder or equivalent safe access must be provided. Scaffold plank must extend over their end supports not less than 6 inches or more than 18 inches The poles, legs, or uprights of scaffolds must be plumb and securely and rigidly braced to prevent swaying and displacement. A safe distance from energized power lines shall be maintained..

Only Authorized and Qualified workers shall clear scaffold of snow, ice, etc. Scaffolds shall not be moved while employees are on them. Loose materials, debris, and/or tools shall not be accumulated on scaffold which could cause a hazard. Scaffolds and components shall be inspected at the erection location. Scaffolds shall be inspected before each work shift, after changing weather conditions, or after prolonged work interruptions. After inspection, the proper colored (Green-ready, Yellow-restrictions, Red-don't use) scaffold tag will be dated, initialed & placed near exit/ egress area Yellow tags will have restrictions written on them Casters and wheel stems shall be pinned or otherwise secured in scaffold legs. Casters and wheels must be positively locked if in a stationary position. Tube and coupler scaffolds shall be tied to and securely braced against the building at intervals not to exceed 30 feet horizontally and 26 feet vertically..
