Introduction  and Objectives Diabetes is a chronic condition characterized by high levels of blood sugar. As a group, we have observed and assessed the negative social conditions in Bhambayi. We also developed a health promotion intervention to improve these conditions.

Methods We found a General Practitioner. We observed consultation at a general practice and picked a diabetic patient for an interview. With appropriate consent we visited the patient in their home environment to determine how the social determinants of health affect the patient and the prevalence of diabetes in his community.

Discussion  Bhambayi is experiencing poverty and physical inactivity Due to poverty, the people cannot afford medication and balanced diet, hence they experience malnutrition Due to physical inactivity, some community members are obese Bhambayi experiences air and water pollution are due to local chemical industries

Recommendations Home and community gardens to address physical inactivity, poverty and malnutrition. Community awareness programs to prevent and manage the disease. Mobile clinics to regularly monitor patients with chronic diseases. Legislation to prevent chemical exposure and pollution which leads to other sicknesses. Educational programs on chemical exposure to avoid other sicknesses.

Results 51 years male with type 2 diabetes mellitus, lives with his partner. Diagnosed in 2018, noticed loss of weight and excessive thirst. Patient on Metformin and was amputated in 2019. No other chronic illness but random acute issues.
