All staff are required to evacuate and go to the designated assembly areas until instruction to return in the building is advised. The activation of a fire detection system is achieved either automatically, initiated by a detector, or through a manual call point or pull station. All staff should familiarize themselves with the locations of these call point. Emergency Staff Identification Designated staff are appointed on each floor of ICLDC during emergency , they can be identified by the following: First Aid Wears an orange tabard with " First Aid" label at the back, they are located at the Nursing Assessment Areas and always ready when emergency arises. In the event that you hear an EVACUATION ALARM or upon hearing the CODE GREEN or CODE BLACK all staff should evacuate the building. Emergency response : Take the exact details of any telephone message ( code words or numbers) Note whether the caller is male/female, has an accent, background noises. Ask the location, size, shape and type of device and when it is likely to go off. The information obtained will make finding the device easier. If you can, while on the call, get the attention of any staff to alert the others. When the call has finished do not hang up, keep the line open (the call may be traced). The evacuation may be limited to a specific area or may be of a largescale, possibly for a prolonged period of time. Every member of staff has the responsibility to be vigilant regarding abusive behavior or assault of patients, visitors and staff happening in the workplace. Code White Response Activation Criteria : Staff perceives themselves or others to be in danger of physical harm or verbally abuse by an aggressive or abusive person. A person is acting out in a manner that is dangerous to self, others or environment. The situation is rapidly escalating out of control. Emergency Management Procedure: • Remain calm. • Speak to the person in a respectful and non-threatening manner. • Then call internal emergency number if possible, or ask other staff to do the call to advise the Operator of the Code White situation. • Be sure to give the specific location where the Code White Response team is needed. • If possible or safe to do so, remove other patients and visitors from the situation..

Contact Maintenance and/or Housekeeping Assist any injured or exposed/contaminated persons. Isolate area &deny entry except to emergency responders. Evaluate the hazard and risk: What spilled? How much? Hazards of the material? Location of the spill? The need to shut down mechanical systems? The potential for environmental contamination? 8. The event of a major center outage, ICLDC has emergency generators that will restore power to some areas of the facility. Emergency Response Remain calm. Follow directions for immediate action. If evacuation of a building is required, seek out persons with special needs and provide assistance. Laboratory personnel should secure all test tubes. Unplug electrical equipment that they are assigned to turn on. All chemicals should be stored in their original locations. • Do not light candles or other types of flames for lighting. • Unplug all electrical equipment (including computers) and turn off the light switches. [Audio] CODE SILVER — WEAPON / HOSTAGE Situation The emergency code designated to alert staff in providing a rapid, organized and thorough response to an incident where there is an individual wielding/firing a weapon and/or a hostage situation. Emergency Reponse Warn others of the situation by calling out for everyone to "take cover". Immediately seek cover or Lock the door to the room. If a landline is available dial the Internal Emergency Number and report " Code Silver". Provide the Operator/Security Dispatch with as much information as possible including, but not limited to the following: - Exact location of the shooting/hostage situation and/or where the individual with the weapon was last seen - Description of the individual - Number of victims and/or hostages - Leave the phone open • If a landline is not available and a cell phone is used, dial " 999" and report that there is a shooting/hostage situation. Provide the 999 Call Center with the following: - ICLDC exact location and the floor / area of the shooting/hostage situation and/ or where the individual with the weapon was last seen - Description of the individual - Number of victims and/or hostages - Leave the phone on but in the SILENT mode.

[Audio] CODE DIAMOND—IT FALIURE Unexpected shutdown and failures are inevitable and therefore an alternate manual process has been created to be followed in any such events.
